Care Data

 The Nelson Practice keeps data on you that may include the following:

  • Your name, address, carers, legal representatives and emergency contact details
  • Any contact the surgery has had with you, such as appointments, clinic visits, emergency appointments etc.
  • Notes and reports about your health
  • Details about your care and treatment
  • Results of investigations such as laboratory tests, x-rays
  • Relevant information from other health professionals, relatives or those who care for you

When you register with us, you will also be registered on a national database held by NHS Digital.

Your data may be shared with other approved organisations who are involved in providing you with care and treatment.

Some of your data is automatically copied to the Shared Care Summary Record.

We may share some of your data with local out of hours, urgent or emergency care services.

Your data is used for national screening campaigns, such as flu, diabetes prevention and cervical cytology.

Your data is used to audit the quality of NHS provider care.

We share data when the law requires us to, for example reporting certain illnesses in the interest of public health, or safeguarding of vulnerable adults and children.

The Nelson Practice handles medical records according to the laws on data protection and confidentiality. Your consent to the sharing of this data within the practice and with others outside the practice is assumed and is allowed by the Law (see Article 6(1)(e) and Article 9(2)(h) of the GDPR).

The Integrated Doncaster Care Record (iDCR)

The iDCR is a new electronic way of storing information about you that can be seen by other health and care staff in different locations in Doncaster when you need help. This means that doctors and other professionals can make quicker and safer decisions about your care. Doncaster CCG has created a leaflet to explain what the iDCR is, how your information is shared, what information is shared and with whom, how these records are kept safe and how you can opt out of sharing your data.
